Show data like a form template

Does anyone have ideas about showing data like a form template in QV? 

Like this:

Header 1	Header 2
Project Name :	Project Code:
Project Manager :	Plan :

Headers are not important. Just want to show data with the structure of rows and columns

I do not know understand it. but you can use text object and show your data like this.

Thanks . I'm looking for special feature or object except text box because when your data is dynamic It can be hard to manage size and appereance

There is no default extension for form. But, samec thing you can achieve like left part is text object and right part should be input object.

I dont think that text object can change size by dynamic data. as you khnow size to data chek box in text object isnt work.
