Sales force integration

Hi Experts,

I'm planning to create a connection for Sales Force in QlikSense.

Can you please let me know what are the necessary/mandatory things to establish connection? 

I know that USERNAME, PASSWORD and SECURITY TOKEN are mandatory.

do we also need Inteface Type, Proxy details to establish  connection ?

I have gone through this link already. but I didn't see list of mandatory fields to connect.
