When reload fails? PLEASE find the below log.

check log for below qv app

Your log shows, there is some script error (i.e. ErrorCount = 3 ) in your application.

As Sunil said, go to the path where the application resides and check for log. If you are not able to find the log activate the log for you application from "Setting Menu --> Document Property--> General Tab --> Generate LogFile". Now try to reload the application and check the log.

if you could post that log here, it would be very useful to debugge.

It looks to me from your log that there is some problem in your "ODBC Connection String" connectivity.

Check whether you have Data Source name avilable in "Control Panel\All Control Panel Items\Administrative Tools\Administrative Tools".

and also check you ODBC drivers.
