Design similar Input Box and Calendar

Hello All,

I need your help in designing and then making the input box work like given below.

What I need :

1) To design the input box with calender object, like the below attached image.

2) When DateMode Filter is selected to be Yearly,  then the calendar object should show Years(say from 2001 - 2012) : Yearly Range

3) When DateMode Filter is selected to be Monthly,  then the calendar object should show Months with Year(like from 01/2001 - 12/2012): Monthly Range

4) What so ever is selected in Start Date and End Date must be displayed inside input box.

5)  Weekly and Daily stuff would get more complex for this level, but would ask when Yearly and Monthly are done.

What I have already done :

1) I created one input box and made hard coded list of Yearly, Monthly, Weekly and Daily.

And I forgot to mention, I am using Personal Edition, users would have to make an extra effort to explain me through steps.
