Displaying table records in a form

I have a table full of customer data. I am looking for a way to display each record which contains customer contact information in a format that replicates an address label:

In the table I have Name, address, phone, contact, contact email.

I would like to display each selected record as:

Name

Address

Phone

Contact

Email

Any ideas on the best way to complete this?
