I want to know which tool is used to create a qlik extensions or themes and how can we preview the file before uploading it through qmc ?

Regarding themes - there is a nice online solution for creating theme (and also preview it before downloading it), give it a try:

Yeah I know this but if I want to create a custom css like I want to add a image as background on my sheets.

Than which tool is supports where I can create json file, css file, java script, html etc..
