Control Charts Question

Hi -

Question - I would like to do a simple control chart which displays the following:

* Line Chart

* Dimension - Month/Year

* Measure : Quantity (example)

* The control would display the average of the filtered data, the upper control limit of the filtered data and the lower control limit of the filtered data

Does anyone have an example of the functions/calculations ?
