need urgently right data model for below xl sheet pls make it all fields are inline and send the screen shot because i am using personal addition ?

i have a given a logical mapping and tables fields mapping pls take all fields as a inline and create a good data model which gives proper relationship . i am greatful to them who help me here pls .....

What is it you are having trouble with?

i Would have a go at creating the data model first yourself and then posting back what issues you face, rather than ask the community to do the work for you.
