Labels on pivot tables

Does anyone know why the Online Sales demo document manages to get column headings for the pivot table shown below for Country, city, State/Province and Postal code, even when the pivot table is collapsed?

My headings only appear when the dimension is expanded. My table is the top table and what I'd like is in the lower table.

If you want to see the labels like in the picture, you have to drag and drop the columns in a horizontal order.

I will send you an example later, im a little bit busy.

if you can find time to send an example that d be great. i just tried to drag and drop, but still they disappear when i collapse the dimesnions.
