Crosstable loading

i would like to find a way to do a cross load of the attached excel sheet.

and get it to load these field into a table

Load

Customer,

Name,

Date, 'hear it would be nice if it created the date from the row info, 2016-1-1 for january etc. (always just the first of the month)

Sales BU,

Margin BU

Is this possible?

And if its possible could the field name change, so if there in the file stood F1 instead of BU.. it would load as

Sales F1

Margin F1

This crosstable load it self are the most important, so if someone only know the answer to that one, please don't hold back
