Table with a weekly and a cumulative count (named columns)?

I have several columns of data in Excel I am pulling into Qlik.

One for "current monthly" that uses a formula, which is last months data when the report is being done

One that names the category

One that names the type

One that names the issue

I want to make a simple table to display: Issue (if "current monthly", if"category", if "type") and the Def% will come later
