Most of it was backing up the settings and applications folder and install the new software. It will require you to uninstall the previous versions, not just upgrade. Details on point 2

By default C:\ProgramData\QlikTech and the folders storing QVWs, Shared, Meta and PGO. More details in https://help.qlik.com/en-US/qlikview/November2017/Subsystems/Server/Content/QlikView-Server/QVSRM_UpgradeMigration11.20-…

No issues with application front-end: triggers, variables, actions, objects, etc. We however did not migrate to the latest version (then 12.20 SR3) because of measurable performance issues in our applications, both in IE Plugin and Ajax and reloading tasks. We reverted to 12.10 SR9 which we are using now and performance is better and closer to 11.20. Anyway, there are changes in the QIX Engine significant enough to understand that resources will be more heavily utilized in different moments than QlikView 11. Reloads in general are faster, but also use significantly more resources than before. We had to move from 15 concurrent tasks to 6, but overall the reloading time is similar.

There are no charges or license changes in order to upgrade to QlikView 12 as long as you have a valid LEF and you are current in maintenance fees. Note that even if you have a valid license number and a current maintenance fee, the PRODUCTLEVEL line must have a date higher than today, else the services will not start or appear unlicensed. If your current LEF shows a lower date, contact Qoncierge to get an updated one, free of charge. (We had this issue).

EDIT: We upgraded from 11.20 SR15 to QV 12.10 SR8 then to SR9. QlikView 12.20 SR3 was used for benchmarking and performance measurements and rejected for the above mentioned reasons. We don't have plans to upgrade to 12.20 anytime soon, definitely not until we can benchmark the newest releases.
