401 Unauthorized after getting the ticket and redirect

I have the following error.
After generating the ticket and redirecting the application to the destination mashup, it shows me the windows authentication screen.
This is a random error, it occurs 10% of the time, using the same user.
What could be happening? Could it be a qlik error?
