That's certainly odd behavior -- I would check the Operations Monitor app to see if the correct app was deleted. In the Operations Monitor app, you can go to the Log Details sheet and then search the table at the bottom to find the logged instances of apps being deleted:
As far as the stream disappearing, it's possible that it was also mistakenly deleted or renamed, but I find it more likely that after the app was moved or mistakenly deleted, there were no other apps left in that stream, which will hide the stream in the Hub, though it still exists and can be found in the QMC.
