I have just upgraded from 2.1 to 2.2 but after the migration I now have two copies of the Operations Monitor (the "2.1" version and the "2.2." version).
I want to delete the 2.1 version but if I go to QMC and select "delete" , then click OK, the app is still there.
How do I delete it?

please check the migration status of the Operations Monitor in QMC--> Apps.
worst case, you can display the GUID of the apps in that screen
--> go to your Apps directory and delete it manually.

I would hold off on deleting the binary as a last resort option because it will not remove the app from the QMC (since that will read from the repository's apps table).
Post the C:\ProgramData\Qlik\Sense\Log\Engine\Trace\ and C:\ProgramData\Qlik\Sense\Log\Repository\Trace System logs since those would be my first places to check for why an app is not being deleted.
