Display Hospitals withing 25 miles of Nursing Homes

I have 2 tables.  One contains Hospital information and the other contains Nursing Home information.  I have addresses for each and I have Long and Lat for each.

I want to create a QS app where I can pick a nursing home and then display all hospitals withing a certain distance. 

Any guidance would be greatly appreciated.
