Phantom of the QMC

Hello Folks,

So I was checking out the Operations log in the Monitoring Apps Stream.  I noticed that I have a failing app.

The only problem is that there used to be an app named TMW_Extractor, but it's been gone for months.

Has anyone else had an issue with a Phantom dashboard loading?  If so, how do I delete it since it isn't necessary any more?

Any help is greatly appreciated.

Normally when you delete an App that has a task, the task is removed when you delete the App.  I have not run into this situation before.

I am assuming that you no longer see the task in the list of tasks?
