reload error in license monitor

Hello! When trying to reload copy of operation monitor of QS server 3.2, getting this error:

Connection 'qrs_userAccess' not found

Server was installed in clean system from zero. All admin users and sa_repository have root admon roles. What wrang?

Also, error log of reloading original monitor is:

      2017-03-03 12:37:21      Error: QVX_UNEXPECTED_END_OF_DATA: Unexpected character encountered while parsing value: <. Path '', line 0, position 0.

      2017-03-03 12:37:21      Execution Failed

      2017-03-03 12:37:21      Execution finished.

I got the same sort of issue after changing Windows Authentication pattern in virtual proxy setting  to forms.(it was Windows). check it back and reload app and see they work
