I am getting this log error when trying the header type connection. Error::Failed to connect to application
I followed step by step the Appendix A and the video on Youtube. I can connect to the application using NTLM.
When I try to connect manually to the hub using a header user I can see the user name in the hub but I get an error message.and when I try to connect to the app I get another error message.
Can anyone help me with this issue? Thanks!

Have you setup your white list on the header virtual proxy to align with how it's configured on the non-header virtual proxy?
It was indeed the white list!
Thanks a lot for your help! I spent an entire day trying to figure that out.
Maybe it would help many other people if this point was added to the tools checklist or documentation.
