Annotations are a server feature. When you create annotations/discussions in a specific AccessPoint document, the actual texts are stored outside of the QVW, just like any other server object. Therefor an example QVW won't do any good because there won't be any trace of annotations in it.
You should see Annotations as a discussion forum/thread on top of a specific document or object in a document in the AccessPoint. Nice for:
discussing data results/anomalies in published QV documents with other stakeholders
inviting other (remote) users to comment on specific results / trends
adding (temporary) comments / explanations to parts of a published QV document
