Export Table to csv with a complex structure

Good afternoon dear friends.

Problem I have the following kind. I need to unload the data table in the CSV (separator ' ; ') But there is one Problem, I have the following structure in CSV:
First line: Table name; Created
Second line: Base table names (as in QlikView)
Third line: Special names (EXW;RTB;SMD;FBV;)
The fourth line is an empty string without records and separators
Fifth row: table data itself

I would be extremely grateful for any help and good all day
