From Date and To Date fields

I would like to include from date and to date , more precisely from week and to week in a table box . 

Can you help me how to do it.

Hi,

Attachment contains table box with week wise demand . 

I would like to see only selective demand within given weeks . So i need a facility to select from and to weeks where upon data from that range would be displayed.

1. It seems you have week wise field, right.

2. And you want to select From Week to To Week on front end.

Can you share a sample in excel file so it understand clearly.
