Help with a condition.

Hello friends,
I am trying to create an alert in Governance Dashboard so that you send me an e-mail every time the recharge of a specific application fails. The drawback that I have, is that I do not know what condition to put on the alert.
The condition should be something like this:
= IF (Task Name) = 'Name of my task' AND (Task Status) = 'Failed'

And this should be fulfilled only for new entries.

Can somebody help me? Thank you!!
