How do I prepare my data for Qlik Maps?

I have a set of data with values corresponding to a date and state.    Currently, I'm not able to use this data in the Qlik maps app.  I'm guessing I need to join it with geocodes.   Hence, I have two questions:

(1)  Where can I get a list of geocodes by US state, MSA, and county?

(2) How do I incorporate this data in my data?
