Need help table design

I have strucked with desiging a table as per business requirement, can any one help me how to design the table as per attached screen. i was trying with Pviot table but i dnt have luck,  so can any one help me on this or is there any other altrenate way to show the same table in QV.

You can use adhoc reporting for this or you can use each straight table for the rows.
