No Ticket Created in Event Log

We are on QV10 SR2, using DMS.  I have two agencies.  One gets into the application successfully.  The event log shows a Ticket Lookup row and then a Ticket created row.  The other agency only displays the Ticket Lookup row with no Ticket Created row.  The message the second agency gets is:

qvp://lossanalysis1.gwccnet.com\Loss Analysis.qvw?iis_authenticate=61F146960F6F5EF70C4E7416C813A4DE6EF790A3 could not be opened

Does anyone know what the problem might be?  The agency is using IE7.
