You seem to be adding a user already. What happens if you now click the Apply button?

Note: in the future, post wider/higher screenshots or reduce your browser window size before making a snapshot. There should be a green +-button in the top right corner, and an Apply-Cancel button combo in the bottom right corner.

It took us a while to get to the correct screenshot. In the future, please post the last screenshot with the actual error message instead of pages that do work.

Check whether the Directory Service log file or the Management Service log file contain any error messages related to this action.

Is there a file called 'CustomDirectoryData.xml' present in folder C:\ProgramData\QlikTech\DirectoryServiceConnector? If so, what does it contain?

The DSC log doesn't tell anything about this problem, and the Custom User DB is not created. OK.

Apparently, the QlikView Management Service has problems storing your Custom Users on disk. Check the QMS log for any messages that have to do with this problem. There should be something...
