Numeric to Text

I'm having trouble getting the Client data to execute in the script. Here is the script I used, but when I reload it says there is an error

Client:

SQL SELECT

      [ContactAddress2],

      [ContactAddress3],

**************the entire script reads

Pharmacy:

Notes shouldn't have a comma
