Hi all
we are facing some issues on Access points.
1. Earlier we were able to see dashboard Icon on Access points. but now we refreshes access points 4 to 5 times. then only Icons appears.
2. After selecting any dashboard we get error "fail to open document, file not found".
3. when we check Audit log, it shows we are accessing bookmark, but no bookmarks are created.
Please help us to resolve this issue.

First check with other browser like IE,Firefox..etc
Better to restart Qv services .
Note:- if possible Restart the server .
so that it will gives you good results.

We tried on IE and chrome but we faced same issues on both browser.
Even we restarted server. but no use.
is it happens for you only (i mean only for 1 user) or for all the users?
confirm with some random users
In QMC, disable "Allow Session Recovery" and try again.
yes it happens with all users
Try to remove cookies in browser settings.
Hope it is a Prod Server:
Check in category section of your access point is showing "ALL".so that definitely it will show all .qvw files as per your privileges.
If it is Dev Server:
Some times it will acts different manner for users so once you restart the server machine you will get the solution.
