Questionnaire in a QV File

Hi All

I have to develop an E'learning application with QV and need to introduce a questionnaire in a sheet with buttons.

The users should read the questions, find the answer in manipulating the others sheets and in last select the response in the questionnaire

Does anyome could send me an example if you have worked on this subject ?
