Section Access locks all ACCESS members except ADMIN

I am noticing a strange behaviour will my application with the following section access table:

After a couple logins for users and the ADMIN without saving the document, the application denies access to the qv document for all users except ADMIN,  why is this happening? Reloading the document allows access again????
