Transforming excel report into Qlik Table

In the attached excel example excel report I have the source that I need to input into qlik and the desired target table in the next sheet. 

Does anyone have any Idea how i can be done? I have come close but still have the Gate that I am unable to have them be repeated in every row.

first in script you have to load the table with "order by" clause and then use the peek function.
