UserId/Password in the same dialog box!

Hello All,

I have the Section Access in my qvw document. Let me know how can i get the userid and password in the same dialog as shown in the below image instead of showing it alone.

That is when using the Ajax ZFP client. Plugin and Standalone QV will have two boxes for user / password.
