Using Google maps at county level in Qlikview

Hi I am new to Qlikview and  trying to use googlemaps for Patients mapping

my data looks like this

Hi Christianj,

Many thanks for your help. Its very good start. I managed to plot on the map but I can't zoom at higher Level. The maximum I am able to zoom as follows

Is there any source that I can download all the Postcodes   Longitude and Lattitudes for UK at county Level (Cambridgeshire, Leicestershire,Northamptonshire etc..). As I can find only Prefix level Longitudes and lattitudes (ex: for Cambridgeshire CB1, CB2,CB3 etc..)
