linking variables in inputbox

Hi All,

I Had an Input box where in there are 3 variables ,If I select the first which is a dropdown second one should populate with data associated to first, and third should be a check box .

What is your actual goal to achieve. Are those 3 variable data not coming from your data model as fields?

Based on variables 1 and 2 ,I want to change variable 3 value by loading a record with these 3 values in to a Database.
