macro export to excel!... help please

Hello  everyone,

I need your help in a problem I am facing. I would like to get a specific format
to the input box variable which was exported to excel sheet. Plus, any
additional new variable input should not replace or delete the previous one. It
should, in fact, be added to the previous one in the same excel sheet 

Kindly find attached the excel format.

Please can you explain in detail. I'm not following it completely. You seem to have two requirements.
