Caption Icons not showing on access point

Weirdest thing, I have ~50 modules on the access point and I have never seen this behavior (screenshot below). Basically, the minmize, maximize, and print icons are not showing, although the export to excel icon is? I've checked all fonts/colors on both doc properties and object properties and they seem to be okay. Any ideas? Although the icons are not visible, if you hover over them you still get the tool tip and can click in the general area to minimize/maximize if needed by guessing.

have you un-check the show min and max option in the caption tab?? if not the case check wheter any other object is over lapping on the same
