converting table in required format

i have following table:

i want to load the data into following format:

ISSUE_ID	Time in Status_STRINGVALUE	Time in Status__NUMBERVALUE	Severity_STRINGVALUE	Severity_NUMBERVALUE	Actual Time Worked (Hours)_STRINGVALUE	Actual Time Worked (Hours)_NUMBERVALUE
