Have you tried updating the license from the QV server? Sometimes that helps. Did you browse the logs to see of there are any clues in there? I have also gone into the properties of each service and manually input the service account information.
I have same licenses on other server. but all services are stopped so there should be no issues. Prior to QV 11 SR 2 I had QV 11 SR 1 and it was working fine (obviously when all QV services on other server are stopped). I am having QVWS running on port 80, nothing else.
