It may take some time as I'm not the direct owner of this environment (I'm just helping another team out). But once we get this sorted I'll provide feedback and let you guys know what the issue was. Thanks for your suggestions.

The issue has been fixed now. I'm told it was that "IIS Authentication Issue", some some clever person had a DENY USERS rule in it.
