Login2.htm "Login Failed" issue with 11.02

HI

We are facing issues while try to login on to the QV Access Portal with the Option "Custom Login Page"

The setting which we have on the server are

#Authentication -> Always

#Type -> Ntlm

#Login Address -> Custom Login Page [login2.htm]

The same setup is working fine while we are using this with 11.00.

Any suggestion or advice helps.
