Internal Inconsistency type d Detected

We are getting internal inconsistency type d detected error in a particular application in QV11 Sr1. We didn't have any problem in QV 9. After the upgrade to QV 11 SR1 we are having this problem.

Most of the time the document reloaded successfully but sometimes we are receiving the above error.

I suspect these types of errors might be handled in V11 SR2, which was just released. I was seeing similar errors in V10 SR4 and they were corrected in V10 SR5.

We are also receiving this message. I have V 11.00.11414.0 SR2. I have noticed this on a laptop with a 32-bit OS, 3 gigs RAM, and an i5 M450 2.40 ghz. My desktop with 64-bit OS, 4 gigs RAM, and Core2 Duo E7500 2.93 ghz so far hasn't experienced this problem.
