Configurable ODBC in Qlikview Server

As Mentioned in QV reference I have configured the ODBC to SQL sqrver.The connection is estableshed successfully and the users <Entity.Name> are listed in QMC to assign CALs.

I have two tables in  Entity (column: EntityID,Name,Descr,email) and Group (column: GroupID,MemberID)

The DSP Settings which I have configured is below

Arun  and Kumar are the users listed from SQL Entity table.

but when I tried to login I am unable to login as it shows an error msg: Login Failed.

From the above SQL table which is considered as Username and Password...?
