How to show 12 months prior data in combo Chart

Hi,

We have a requirement, where we have a bar-line combo chart.

In bar graphs we need to show  current month data(measure will be count of id's), in line chart we need to show 12 months prior data(measure will be count of patient id). 

How to achieve that scenario ?
