In this session, two leaders from the Care Coordination Institute (CCI), which is part of the Greenville Health System and Palmetto Health, will review how they have modernized the organization’s enterprise infrastructure to collect EMR data across a diverse network, combine it with financial data, and deliver it back to those who need it to support value-based programs.
