Tracking and Improving Ambulatory and Inpatient Measures

Learn how Contra Costa Health Services is leveraging a Pay for Performance Dashboard

PRIME (Public Hospital Redesign Incentive in Medi-Ca) is California’s five-year Medicaid Waiver with projects and goals to ensure health systems:

  • provide patient‐centered, data‐driven care
  • advance population health management
  • implement data analytic capacity to drive system‐level improvement
  • improve health outcomes as evidenced by 60+ metrics

The projects associated with PRIME are centered around:

  • Outpatient Delivery System Transformation and Prevention
  • Targeted High Risk or High Cost Populations
  • Resource Utilization Efficiency

The 60 measures involved in the projects include Depression Remission, Comprehensive Diabetes Care, Tobacco Counseling, SBIRT, Controlling Blood Pressure, Colorectal Cancer Screening, Influenza Immunization, Breast Cancer Screening, Cervical Cancer Screening, Postpartum Care, Timely Prenatal Care, Medication Reconciliation. All-Cause Readmissions, etc.

Public Health systems across California, as well as those in other states with similar programs, will need a robust analytics infrastructure to track metrics to meet milestones and report data to maximize PRIME incentive payments that total $3.2 billion.

Dr. Rajiv Pramanik, CHIO and Bhumil Shah, Asst. IT Director at Contra Costa Health Services will share the strategy behind their analytics program, demonstrate how they are leveraging their Epic EMR data and Qlik Sense to make data available for tracking and improving patient care, and report on the results realized for their organization in just a few short months.
