It sounds simple enough but you aren't giving us much detail to go on. What do you imagine will be the challenging aspects of the report? For something so simple (you've only described two fields thus far) perhaps a BI tool such as Qlik isn't needed at all, it could be done simply with a database script. How do you want to deliver it or have it consumed? Do we assume you just want to do it in Qlik Sense and not QlikView or with NPrinting?
