Dynamic Pivot Tables

Hello,

Is anyone able to explain how to create a dynamic pivot table is shown in the following example?

You're able to select the metrics as well as the dimensions and the pivot table populates based on your selected inputs. I'm unsure how to create the dimension and metric checkboxes and in turn how to turn the table into a pivot table. Do you create a container consisting of a pivot table, dimensions list box, metric list box, etc?

Any insight is much appreciated.
