Is there a way to determine if a Theme has been applied to a document, sheet, or object? If so how can we know what Theme it is?

You can find out whether theme is applied to your sheet or not by:
Goto Sheet Properties --> General tab.
In Tab Settings, you can see that custom color checkbox is checked.
In Background, you can see that under sheet settings colour or image is set to the document.
In Tab Settings, the custom color is a calculated expression. Hence, you can also modify the colour and customize the theme and create your own theme and save it for further re use.

Yes, I can see in Sheet Properties - General tab what the background or tab color has been set to, but this doesn't tell me if a Theme has been applied and what the name of that Theme is. Is this possible?

Here is a partial answer to my own question:
The default theme can be set at the User and/or Document level - any settings made here can be seen in the following places:
1) User Preferences -> Design -> Default Theme dropdown, or
2) Document Properties -> Presentation -> Default Theme for New Objects dropdown.
I’m still researching whether there is a way to determine which (if any) Theme a sheet or an individual object is tied to. Can anyone provide the answer?
