Qlikview - Document Failed to Load

Hi Qlik Community,

I am facing an issue where I am trying to open a QVW but an error message "The document <doc name> had failed to load" and get the additional error code:

I am running on Qlikview 12.40.20300.0 and I have tried reloading with no data (both front end and command prompt) but always the same error message.  What is really strange is that I have a number versions of same document (some 3/4 years old) and none of the documents will open when they previously did.  All my other documents open with no issues and only this one does not open.  

The document was correctly open in September (when it was last updated) and the only thing that has happened since the last update was that we updated from Qlik 11 to current version?

Anyone any ideas how to correct this? Even if I can say the layout it would help greatly.
